Requesting Anniversary Certificates

There are two production runs per year for clubs who required 50 Years Continuous Service Award, Ninety Year Recognition and Centenarian Recognition certificates.
Please see the production cut-off dates below. 

What do you need to do?

1. Check the cut-off dates to make sure production is open. Clubs who miss the below production cut-off dates will unfortunately need to organise their own certificates for their members independently.
2. Fill in the excel spreadsheet provided by Bowls Victoria. Click here to access this form.
3. Enter ALL requested details.
4. Email the completed excel spreadsheet to bowlsvic@bowlsvic.org.au by the production cut-off (see dates below)

Please take care with spelling and ensure names and addresses are spelt correctly. Please also ensure you write in sentence case and not in capital letters.
